Referral Exchange aims to digitise all healthcare referrals in Singapore, starting with subsidised specialist referrals. It is a backend layer that integrates with all major healthcare IT systems and consolidates all the API endpoints, so that they are easily consumable by upstream systems.

Q2 2025 Apr-Jun

  • Simplified API design to support a harmonised, consistent experience for referral creators.
  • Continued to work with clusters to optimise referral tips.
  • Worked closely with Synapxe, AIC, MOH on building integrations with other healthcare systems.
  • Worked on interim solution for AAC referrals from Bright.
  • Worked with MOH to scope out a pilot to optimise GP referrals for diabetic foot screening, diabetic retinal photography and nurse counselling services.

Q1 2025 Jan-Mar

  • Completed user research led by Design and Prod Ops to better understand GP behaviours and attitudes towards SOC referrals).
  • Automated follow-ups with SOCs for pending referrals, saving ~30 mins/week and improving response consistency.
  • Implemented offering-specific description property to support contextual ‘referral tips’ for GPs.
  • Completed SDK and API documentation to support CMS vendor integrations, reducing manual verification work.
  • Contributed technical and user journey inputs to AAC, ACP, and CMH referral flow design through workshops and tabletop exercises.

Q4 2024 Oct-Dec

  • Onboarded PHI SOCs across 3 clusters, and all GPs who are using HAS to properly stress-test the schema-based referral approach.
    • Implemented email acknowledgment workflow to support digitising referrals needing triage.
  • Implemented clarification notes feature to support closed-loop communications for referrals needing triage.
  • Implemented attachments feature to support referrals needing clinical attachments, e.g. chest x-ray and ECG reports.
  • Worked closely with clusters to improve referral accuracy and follow-through.

Q2 2024 Apr-June

  • Pilot to allow GP clinics to make digital referrals to 8 Polyclinics under NHG for critical diagnostic services
    • Polyclinics onboarded: AMK Polyclinic, Geylang Polyclinic, Hougang Polyclinic, Kallang Polyclinic, Sembawang Polyclinic, Toa Payoh Polyclinic, Woodlands Polyclinic, Yishun Polyclinic, AMK-Thye Hua Kwan Hospital
  • Fully digitised NHG Diagnostics’ Radiology and Laboratory Referral Forms, so GP doctors do not need to fill out a manual form
  • Fully digitised MOH’s CHAS referral form for subsidised specialist referrals, so GP doctors do not need to fill out a manual form
  • Improved the user experience for subsidised specialist referrals for patients not in SingHealth’s patient records

Q1 2024 Jan-Mar

  • Pilot to allow GP clinics to make digital referrals to more than 300 SingHealth Specialist Outpatient Clinics (SOCs) for specialist treatment
    • Hospitals onboarded: Singapore General Hospital, Changi General Hospital, Sengkang General Hospital, KK Women’s and Children’s Hospital, and National Dental Centre Singapore
    • Completed integration with SingHealth’s PartnersBuddy to access SingHealth SOC appointment slots
    • Completed penetration testing with Cure53 to ensure system security
