Why we built this

MSWs need to spend time on administrative tasks, beyond patient care. Along with fragmented patient data, this created additional strain on MSWs when providing holistic care.

What we've built

Care360 is a patient management system that reduces manual work, and provides access to holistic data for MSWs. This way, you or a loved one can receive the most effective support when it matters most.

Patient referral management

A single platform does it all. MSWs can now receive and monitor referrals to attend to patients’ financial, emotional and social needs promptly.

Data sharing across institutions

MSWs can now access up-to-date patient data across institutions, so patients don’t have to repeat and relive difficult situations to get care.

Integrated financial applications

Submit, track, bill—every step of financial applications is built in. This means approvals move faster for straightforward cases, providing valuable assurance for patients.
